What information do you need for an EAA referral?
We need the client’s name, address, contact information, waiver program and ID number, your contact information, a brief description of the client’s mobility limitations or medical conditions, and any specific concerns or requests. The more detail you provide upfront, the faster we can schedule and complete the assessment.
Do you provide status updates during the approval process?
Yes. We proactively communicate with you at each stage—assessment scheduled, assessment completed, report submitted, and any follow-up requests from the waiver program. You’ll never have to wonder where things stand or chase us for updates.
